In this video, Amy E. Krambeck, MD, discusses incorporating emerging technologies into her holmium laser enucleation of the prostate (HoLEP) practice for the treatment of benign prostatic hyperplasia. Krambeck is a professor of urology at the Northwestern University Feinberg School of Medicine in Chicago, Illinois.

How do you incorporate emerging technologies into your HoLEP practice to enhance efficiency and improve patient outcomes?

I'm always trying new things. I feel like, once you decide that this is the way you have to do it and stick with that forever, that's when you immediately become irrelevant. Technology is constantly changing and evolving. So I'm constantly trying new video systems, new scopes, new lasers, etc, and seeing what works and what doesn't. There's really no good disposable scope for HoLEP yet. We do try varying size scopes. And we actually did a prospective trial where we compared 24 Fr, the smaller scope, to the 28 Fr larger scope, and really found no benefit in either one. It really has to do with the preference of the surgeon, and how they can be most efficient. I think probably the biggest improvement that has improved efficiency is the Wolf morcellator, which is very fast and efficient. I'm sure we'll see some upgrades on that in the future, or maybe even new morcellators on the market. And then the pulse modulation with the laser has also been very helpful with that. However, the newer ureteroscopes and cystoscopes, they've been huge improvements for stone disease, but not so much for BPH yet.
